SERVICES & FEES

Neuropsychological Evaluations:

• $350 per hour – A selected battery of neuro-cognitive, IQ/intelligence, academic, visual- motor, executive function, memory, and personality measures.

Psycho-Educational Evaluations: 

• $350 per hour – A battery of cognitive, academic and pathology measures. Extended or additional measures may be used, as needed or determined by the clinician. Standard evaluation completed privately for a school student (school district, private school, etc.) and accommodations on standardized tests (SATs, GREs, GMATs, LSATs, MCATs, Board Exams, Bar Exam, etc.)

Psychological Evaluations: 

• $500 – Consult (fee waived if testing or evaluation is agreed upon and completed)

• $350 per hour – Psychological interviews (clinical consults, legal/forensic cases, therapeutic need, employee or workplace accommodations e.g. fitness of duty, mental health clearance, testamentary).

• $350 per hour – Forensic and legal cases; interview, testing, and report. Each case is determined on a need and case by case basis with the individual, attorney or law firm. An expert witness rate sheet is provided on request.

Psychotherapy and Consultation:

• $250 per hour – psychotherapy for adolescents and adult specific to the need of the individual.

• $450 per hour – testimony services, with a four-hour minimum.

• $450 per hour – deposition and testimonial services (includes research and preparation).

***All fees must be agreed upon and satisfied prior to testing.***

Rate quotes vary based on individual need as expressed by patient or student, ascertained by the clinician and requirements of the school, program or facility requesting the evaluation and report. Rates are inclusive of the interview, assessment, testing, and report. Extended consults after the provision of the report may be subject to additional fees. Rates are negotiable upon agreement by Dr. Grajewski and patient.

Lisa J. Grajewski Clinical and Forensic Psychology, Inc. prides itself on immediate scheduling for evaluations and a 2-3 week period for reports on evaluations upon completion of testing. In some cases, for additional fees, reports can be expedited to meet deadlines.

You may pay by Cash, Check, Visa, Mastercard, or Venmo.

Insurance

We do not accept any insurance but will provide a super bill for you to submit for reimbursement claims.

cancellations

48 hours notice is required for cancellations or rescheduling of appointments, otherwise the full fee will be charged. However, when more than 24 hours notice of cancellation is provided, we will endeavor to offer your appointment time to another client, and if the session is re-allocated no fee will be charged.
